Citizens can access Birth/Death Certificates based on the request submitted in MeeSeva centers.The certificates will be issued instantly from Hospitals & MeeSevacenters.
And also citizen can get already registered Birth/Death Certificates through GHMCApp and GHMC website. Below are the URLs

BIRTH & DEATH EVENT AT HOSPITAL
Each Hospital has to submit the form no-1(Birth legal information ) & form no-2 (Death Legal Information) with signed and stamp of the Hospital from time to time to the GHMC within the (7) days from the date of occurrence of the Birth & Death events.
•INSTITUTIONAL Delivery / Death at Hospital Request will come through hospital online to Sub –Registrar/AMC.
•NON INSTITUTIONAL Delivery / Death at Home other place Hospital Citizen approach the MeeSeva center along with all required documents including form-1 & Form -2, then the certificates will be issued instantly from MeeSeva centers. After approval by Registrar/AMOH and SMS alert send to Citizen.
•For certificate citizen will approach to any Mee-Seva
•Pay the Fee Rs.45/- and Dispatch through Mee-Seva to applicant.
BIRTH AND DEATH PROCESS (FLOW IN MODULE)
•If the Birth & Death occurred in hospitals, the Hospital people directly enter the Birth & Death events into the online and after verification of Registrars of Birth & Deaths approval will be given later on, citizens can take such Certificates from Mee-Seva center only.
•In case of Home Birth, the relatives of Birth person should take immunization card from concerned Hospital and they have to approach Mee-Seva center for apply Birth Certificates. After updating they have to take Birth Certificate from Mee–Seva centers.
•In case of Home Death, the relatives of deceased person should take burial ground receipt from the concerned burial ground/ register deceased details in burial ground App After updating they have to take Death Certificate from Mee–Seva.
